Where can I find my login information?

If you are a person to whose cellular telephone number Molina Healthcare, Inc. (“MHI”) or Molina Healthcare of California (“MHC”) (MHI and MHC are together referred to herein as “Defendants”), or Icario, Inc. on behalf of either Defendant, placed one or more artificial or prerecorded voice calls relating to a MHC or MHI plan or account that is not, or was not, your own plan or account, between May 30, 2021, and April 21, 2026, you may be entitled to compensation as a result of the Settlement.

The Settlement will result in the creation of a non-reversionary $1,927,500.00 fund to fully settle and release certain claims of persons to whose cellular telephone numbers either Defendant or Icario, Inc. on behalf of either Defendant, placed one or more artificial or prerecorded voice calls relating to a MHC or MHI plan or account that is not, or was not, that person’s own plan or account, between May 30, 2021, and April 21, 2026.

The Settlement fund will be used to pay Settlement amounts to approved Settlement Class Members who elect to participate in the Settlement, after deducting the costs of Settlement notice and administration, attorneys’ fees, litigation costs, and expenses, and an incentive award to Plaintiff Melissa Kruzel, the consumer who initiated the class action lawsuit against Defendants.
